There are millions of places where you can buy a CMOS video camera.
Try, for example, RADIOSHACK:

I have been able to remove the video chip from a Grundig video camera. Anywhere I can find information as to how it works and to possibly reuse it. I don't know the part number of the chip.

Hi Maverickmax,
If you are in the US, there is a cheap one-time-use digital camera in CVS/Phamacy store. It has everything you need- image sensor, optics, uC, RAM, even a LCD. A camera costs $20 and CVS expects you to return it for digital films developed. However you can easily open it up and do whatever you want. Google "CVS camera h(at)ck" for more information.
